Job Description

Hire, train, supervise and schedule the care staff members as well as coordinate all services relating to resident care needs in the memory care neighborhood (and AL unit as needed or as determined by the supervisor or Clinical Corporate Team). Manage resident changes of condition in coordination with Resident Service Director. Audit the medication administration program to assure accuracy and completeness. Address resident and family concerns, respond to resident special requests, and act as a liaison within the community with other staff.

Job Functions

Essential Functions:

  • Hires, trains and supervises memory care team members.
  • Responsible to participate in coordinating training and orientation for all new care staff.
  • Scheduling and Appropriate staffing of care staff in collaboration with the RSD and ED. Constructs and provides a schedule for the MC clinical care staff as assigned, making sure there is coverage on every shift (and supports AL neighborhood as needed). All overtime hours must be pre-approved by the RSD & ED.
  • Develop the initial monthly clinical schedule for each community and ensure all shifts are filled for the month. Have this developed and posted at least 10 days prior to start date.
  • Provide these applications to the RSDs at each community based on needs.
  • Review staffing patterns monthly with Executive Director/RSD and makes an adjustment based on census.
  • Provides care staff coverage as needed when unable to fill a shift when a call-out arises
  • Maintains attendance records, and tracks and logs attendance for all clinical staff.
  • Participate in all in-service programs to expand and develop abilities.
  • Maintains compliance with all personnel policies, established community policies and procedures, and Federal and State regulations and standards, including but not limited to HIPAA privacy and confidentiality laws.
  • Provides direct resident care as needed based on the needs of the community. Functions as a caregiver when assigned including monitoring and responding to resident call response system.
  • Develops and maintains courteous and professional relationships with residents, visitors, and co-workers.
  • Dependable and able to meet schedule requirements (attendance and punctuality) with no restrictions.
  • Coaches and counsels employees, exercise disciplinary action by oral and written communications and coordinates all actions with the Resident Service Director and Executive Director.
  • Evaluates employees regularly and annually.
  • Attends daily standup meetings, reporting any resident issues as documented in the 24 Hour Report or Incident Reports.
  • Monitors resident care needs, alerts the Resident Service Director and resident's health care provider when the resident has a change of medical condition.
  • Reviews all medication orders daily and follows up on medications that were not preauthorized in resident's Progress Notes.
  • Attends weekly Service Plan meetings. Assures input from care staff and completes Service Plan review notes in the resident's clinical record.
  • Checks the Service Plans regularly for specific resident's needs. Assures follow through with the services as described in the Service Plan. Reports any discrepancies to the Executive Director and Resident Services Director. Charts vital signs taken during shift on any resident in the resident's Service Notes. Reports any changes in resident needs or conditions to Resident Service Director. Reports any tasks needing to be passed on or followed through to off-going and on-coming shifts.
  • Establishes a monitoring process for "at risk" residents and assures follow up charting.
  • Follows up on Incident Reports as directed by the Executive Director.
  • As directed by physician orders and Resident Service Director, administers and orders medications as needed by residents for whom we provide medication administration services.
  • Monitors all ordered supplies.
  • Coordinates resident appointments when required.
  • Reviews all physician orders for accuracy. Assures resident's physician orders are reviewed and signed periodically.
  • Audits the Medication Administration Record weekly. Document findings providing a copy to the staff and the Resident Service Director or Executive Director.
  • Assists with the new resident move in process. Assures initial resident paperwork and physician orders are on hand. Ensures Engaged Day is completed with family/POA prior to move in. Communicates to the care staff the new resident's needs and preferences. Assures the dining staff has been informed of diet ordered, food allergies or special dietary needs prior to resident's arrival for the first mealtime.
  • On call 24/7, 7 days a week. While on call, responsible to triage emergency care needs, assures staffing needs are met and responds to other miscellaneous problems that may arise in the whole Community (MC and AL).
  • Performs Quality Assurance Audits and support the whole community/AL side, as directed by RSD or Clinical Corporate Team.
  • Assures resident-specific activity interest/ability assessment is conducted and included in the resident Service Plan.
  • Supports daily schedule of life enrichment activities.
  • Conducts monthly trainings to Memory Care staff.
  • Conducts/ coordinates the delivery of ongoing monthly support groups for resident family members.
  • Responsible for monitoring employee hours worked and staffing patterns to assure the Memory Care Unit remains within projected budget.
  • Responsible for monitoring weekly the declining balance sheets for all departments to assure the Memory Care Unit remains within projected budget.

Non-Essential Functions:

  • Attends all required training, in-service, and staff meetings.
  • Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
  • Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
  • Adheres to all policies and procedures of Aventine Hill Communities.
  • Acts as support structure on the AL side as the need arises and performs other duties as assigned by the Resident Services Director or Corporate Clinical Team.

Qualifications/Skills/Requirements

  • Currently certified and willing to maintain CPR Certification (if required by regulations/policy) and First Aid.
  • Recent history of care experience in memory care setting.
  • Recent history of medication assistance experience in memory care setting.
  • Excellent customer service and communication skills.
  • Positive and cheerful disposition.
  • Ability to coach and mentor.
  • Proven leadership qualities.
  • Proven organizational qualities.
  • Comfortable making decisions and directing others and resolving conflict.
  • Knowledge of progression and care of dementia residents.
